All summer students contracted with Residence Life will be housed as assigned by Residence Life—summer conferencing services may prevent assignment to current or fall assigned rooms.  Currently, Residence Life is planning to house all SDSM&T students on Peterson 3rd floor in double and deluxe double rooms. Costs listed are for Summer 2007, expect 2008 rates to be higher.  If you stay less than 4 weeks, the SD use tax must be added to your costs.

NOTE: To ensure roommate preference, please return applications to Residence Life together. You must mutually and exclusively request one another. 

   I would like a DOUBLE ROOM ($72.20 per week).
   I would like a SINGLE ROOM ($88.85 per week). Single rooms are assigned on space available basis at a premium rate.

Special Circumstances
I have special physical/medical circumstances of which Residence Life should be aware. Please briefly explain (this may include diabetes, allergies (include allergies to smoke), being taller than 6’4”, etc.) All information will be maintained confidential.

COEDUCATIONAL VISITATION POLICY: All halls have 24 hours visitation for SDSM&T students during the summer months (May 10 – August 15). All guests must be escorted at all times.  Guests staying overnight for more than two days must be registered and will be charged for lodging. 
ALCOHOL, MARIJUANA AND CONTROLLED SUBSTANCES POLICY:
(Board of Regents Policy Manual 3.4.2.b.16.) The manufacture, sale, possession, use or consumption of alcohol, marijuana or controlled substances by students on any property controlled by the Board of Regents or used in connection with any institutionally sponsored activity; except that alcohol may be served at social activities held in other locations subject to the restrictions set out in this article. 
BOARD OF REGENTS SMOKE FREE POLICY:
All residence halls have been designated as "limited smoking" facilities. Per vote of SDSM&T residence hall councils, all residence halls at SDSM&T are smoke free including individually occupied rooms. 
Summer Housing Agreement
In submitting this form, I am certifying the accuracy of all information provided. I further understand that late notice of cancellation of this agreement may result in administrative costs being assessed to my student account. I am aware of and understand that I must abide by the rules and regulations of the SDSM&T Residence Life as outlined by the SD Board of Regents and the university. I give authorization to release my address and phone number to any assigned
roommate(s). I understand I will be charged a weekly rate for the duration of my stay and payment is due at the SDSM&T Cashier’s Office based on the schedule agreed upon.
